Social Determinants of Health

What are the social determinants of health and how do they affect our patient populations?

The Social Determinants of Health are “the structural determinants and conditions in which people are born, grow, live, work and age.” These factors include socioeconomic status, education, the physical environment, employment, and social support networks, as well as access to health care. In the video presented here, we will introduce you to the social determinants of health that are more specific to the patient population we serve at Olive View. We hope that as you care for your patients, you will keep these important determinants of health in mind, allowing you to better tailor your approach to patient care in our underserved population.
