  • Take the Roxford St entrance to the 210 North toward Sacramento.
  • Stay right to merge onto the 5 South toward Los Angeles.
  • Stay middle-right to merge onto the 405 South toward Santa Monica, and continue about 6 miles.
  • Take the Sherman Way/Reseda exit.
  • At the freeway exit, turn right onto Haskell Ave.
  • Turn right onto Saticoy St and continue 1.5 miles.
  • Turn right onto Van Nuys Blvd.  Mid-Valley CHC is immediately on the right.
  • Turn right into the driveway and parking structure, where you can park.

  • Wear your Olive View ID badge through the main entrance.
  • Take the employee stairwell to the 2nd Floor.
